Ask your physician if you qualify for the Limited Use (LU) codes (such as 665, 666, or 667).
Talk to your doctor. Ask: "Have I tried Metformin? If not, is there a medical reason I can't take it?"
Your doctor must include the appropriate LU code directly on your prescription for the pharmacy to process it through your ODSP drug card.
